Where: LTO-BALAMBAN LICENSING EXTENSION OFFICE
Requirements for Plate Claiming:
For Registered Owner:
•Photocopy of OR and CR
•Photocopy of Valid ID
For Second Owner:
•Photocopy of OR and CR
•Photocopy of duly notarized deed of sale
•Photocopy of Valid ID of both Vendor and Vendee
For Authorized Representative:
•Photocopy of OR and CR
•Original authorization letter from the registered owner.
•Photocopy of Valid ID of both the registered owner and authorized representative.
